Connecting People to Community: A Social Prescribing Approach

On Social Prescribing Day (Thursday 26 March 2026), Chatty Café Australia is highlighting its 2026–27 Pre-Budget Submission to the Australian Government – a proposal that sets out a practical, community-led approach to addressing loneliness and social isolation in Australia.

Social Prescribing Day is a global celebration of the people, organisations and communities working together to connect individuals with non-medical support to improve health and wellbeing. It reflects a growing international shift toward more collaborative, community-based approaches to care.

In Australia, many people experiencing loneliness do not identify as needing “help” and are unlikely to access clinical or formal services. This creates a critical gap – where early indicators of social isolation are present, but pathways into connection are unclear or inaccessible.

Chatty Café Australia’s submission proposes a scalable, community-embedded social prescribing pathway that enables early, upstream intervention – starting in one of the most trusted and accessible settings in the country: the local pharmacy.

The model brings together:

  • Pharmacists as trusted connectors, able to identify early indicators of social isolation through everyday interactions
  • Link Workers who provide navigation, coordination and follow-up
  • Volunteer Buddies who enable a safe, non-clinical introduction into community
  • Partnerships with existing community venues and activities, including Chatty Cafés and other local initiatives

Together, this creates a supported pathway for people who are not actively help-seeking – enabling safe, timely connection into community before loneliness escalates into crisis or clinical need.

Importantly, the approach builds on what already exists. It strengthens connections between primary care, volunteers and community infrastructure, rather than creating new systems.

This work reflects growing collaboration across sectors (including community organisations, researchers, primary care professionals and government) to better recognise and respond to loneliness as a public health priority.

As Social Prescribing Day highlights, connection is not just a “nice to have” – it is essential to individual and community wellbeing.
